Coast Dental Services, Inc. is a management company that provides non-clinical administrative and business services to dental offices.

         REGISTERED INSURANCE PLAN.
2.     This plan is not valid with any other offers, promotions or dental
         insurance.
3.     Smile Plus is only valid at Coast Dental offices.
4.     Prices are subject to change without notice.

Minimum fee only.  THE PATIENT AND ANY OTHER PERSON RESPONSIBLE FOR PAYMENT HAS THE RIGHT TO REFUSE TO PAY, CANCEL PAYMENT, OR BE REIMBURSED FOR A PAYMENT FOR ANY OTHER SERVICE, EXAMINATION OR TREATMENT WHICH IS PERFORMED AS A RESULT OF AND WITHIN 72 HOURS OF RESPONDING TO THE ADVERTISEMENT FOR THE FREE, DISCOUNTED FEE, OR REDUCED FEE SERVICE, EXAMINATION OR TREATMENT. ADDITIONAL CHARGES MAY BE INCURRED FOR RELATED SERVICES WHICH MAY BE REQUIRED IN INDIVIDUAL CASES. 

Coast Dental Advantage® Notices and Disclaimers

Monthly No Interest Option with Regular Minimum Payments
Special terms of the 6 and 12 months no interest option with regular minimum monthly payments will apply to charges made with approved credit to your Coast Dental Advantage credit card issued by Wells Fargo Financial National Bank.  Regular minimum monthly payments are required during the option period.  The no interest option means there is no interest if your purchase is paid in full within the 6 or 12-month period following the date of the purchase; otherwise, interest accrues from date of the purchase at the regular APR. The regular APR is 23.9% as of March 1, 2009. The APR may vary quarterly.

Reduced Rate With Regular Minimum Payments
Special terms of 8.9% (.742% monthly periodic rate) with regular minimum monthly payments will apply  to purchases charged with approved credit to your Coast Dental Advantage card issued by Wells Fargo Financial National Bank.  Regular minimum monthly payments are required. The regular APR is 8.9% as of March 1, 2009.    The default APR is 23.9%. All APRs may vary.
